FAIR VALUE REPORT
UNITEDHEALTH · UNH · OCT 17, 2026 · AFTER RESULTS OF OCT 13, 2026
UnitedHealth beat by 55 cents. Why did three banks cut their targets anyway?
Adjusted EPS of $4.69 crushed the $4.14 consensus and the 2026 floor rose to $20.50 - yet targets fell. The model says $460, 14.6% above the price. The catch is where 53 of those 55 cents came from.

THE NUMBER IN 3 POINTS
- 01UnitedHealth's $4.69 adjusted EPS beat the $4.14 consensus, but $610 million of reserve releases added about $0.53.
- 02The FPI fair value is $460.06, 14.6% above the $401.51 price, versus a $469.10 Street mean and $491 median.
- 03Medicare Advantage membership fell 935,000 to 7.605 million by 30 September 2026 as margins rose to 3.3%.
A 55-cent beat, 53 cents of which came out of reserves
UnitedHealth $UNH reported adjusted earnings of $4.69 a share for the quarter to September 2026 on 13 October 2026, against the $4.14 LSEG consensus tracked by Reuters. Revenue of $112.7 billion was slightly below the $113.2 billion of the quarter to September 2025.
The quality of the beat is the issue. The medical care ratio - the share of premiums paid out as claims - fell to 88.4% from 89.9%, but the 8-K filing shows $610 million of that came from reserves set aside earlier in 2026 that were not needed. That is worth about $0.53 a share after tax. Strip it out and the underlying quarter landed on consensus.
The 2026 outlook moved to at least $20.50 from the $19.50-20.00 range set in July 2026 - roughly the reserve release passed through. Nine-month adjusted EPS is $18.30, up 29% from $14.22, so the implied fourth quarter is only about $2.20 against $2.11 in the quarter to December 2025. The FPI consensus for that quarter is $2.27. Growth is about to slow from 29% to single digits.
Street vs model: three cuts, three $500 targets, a $460 model in between
Three banks lowered their targets within 48 hours of the beat. Raymond James went to $450 from $475 on 15 October 2026 while keeping Outperform, applying about 16x 2028 earnings - a multiple it shrank because of the 3 November 2026 midterms and possible changes to Medicare risk adjustment and Star ratings. Baird cut to $427 (Neutral) and Barclays to $440 (Overweight) on 14 October; TD Cowen held at $402 on 15 October. On the other side, Piper Sandler raised to $504, and Bernstein, Stephens and H.C. Wainwright all posted $500 on 16 October.
The FPI data show the drift: 17 targets issued in the last quarter averaged $483.88; the 8 issued in the last month average $465.38. Consensus is still 43 buy, 7 hold, 2 sell, with a mean of $469.10, a median of $491 and a range of $373-$529.
The FPI fair value of $460.06 sits 1.9% below the mean and 6.3% below the median - almost exactly where the fresh targets are landing. The gap is not about earnings. Management pointed to the low end of its 13-16% long-term growth range as the starting point for 2027, which on $20.50 means $23.17-23.78, above the $22.60 the Street carried before the report. The dispute is over what multiple to pay for those earnings.
Why the model stops at $460: 22x the 2026 floor, 20x the 2027 low end
The DCF half of the model rests on cash the company already generates: operating cash flow of $25.1 billion in the nine months to 30 September 2026, a free cash flow yield of 6.25% (annual cash left after investment, divided by market value), and $5.0 billion of buybacks plus $6.2 billion of dividends in the same period. Cash and short-term investments stood at $28.5 billion against $5.6 billion of debt due within a year and $68.7 billion of long-term debt.
The relative part (30%) is where the discount is. At $401.51 the stock trades at 23.0x earnings against an industry median of 24.0x, and at 12.2x EV/EBITDA against 13.9x - while its net margin of 3.5% and return on invested capital of 9.8% beat the industry's 2.3% and 6.8%. The market is pricing the best operator in the group below the average one. A P/E primer explains why that comparison matters.
History frames the rest: EV/EBITDA was 18.6x at the end of 2021 and again at the end of 2024; now it is 12.2x. The $460 fair value equals about 22.4x the $20.50 floor for 2026 and 19.9x the low end of 2027 - below every year-end multiple from 2021 to 2025.
935,000 fewer Medicare Advantage members, Optum Health at 3.2% on a 6-8% target
Earnings are growing because volume is being given up, not won. Medicare Advantage membership was 7.605 million at 30 September 2026, down 935,000 since 31 December 2025, and Medicare & Retirement revenue fell to $42.4 billion from $43.4 billion. UnitedHealthcare's operating margin still rose to 3.3% from 2.1% - the landlord who evicted his worst-paying tenants and collects more from fewer doors.
Optum Health tells the same story: revenue down 10% to $22.9 billion while operating earnings jumped to $735 million (a 3.2% margin) from $41 million. The long-term target for that unit is 6-8%, so less than half the recovery is done.
The ceiling for 2027 pricing is set: CMS finalized a net average Medicare Advantage payment increase of 2.48%, over $13 billion industry-wide, on 6 April 2026, and kept the 2024 risk-adjustment model. UnitedHealth has at least 69% of members in 4+ Star plans for 2027. What the Street is pricing is the risk that politics changes those terms after 3 November 2026.
What moves the stock before 26 January 2027: $23.17, 88.4% and $359
The 2027 outlook arrives at an investor event before the end of 2026, ahead of the 26 January 2027 results. The up case: a 2027 figure at or above $23.17, plus a fourth quarter above the $2.27 consensus. At $460 that would be about 20x forward earnings, in line with the 2023 multiple, and the $491 median target would be within reach.
The down case: a 2027 outlook below $22.60, a medical care ratio back above 89.9% once reserve releases stop, or a post-midterm policy move on risk adjustment that pushes the Street toward Raymond James's 16x. At 16x the low end of 2027, the stock would be worth about $371 - close to the $373 low target.
Three signals to watch:
- the 200-day average at $359.10 and the 50-day at $387.30 - the stock sits above both;
- the medical care ratio in the quarter to December 2026: anything near 88.4% without reserve help confirms the margin is real;
- Optum Health's operating margin holding above 3% - the segment the 6-8% target depends on.
The 14.6% gap to fair value is a bet that the multiple stops shrinking, not that the earnings keep surprising.
